THE OPPORTUNITYThe Patient Care Coordinator is a healthcare professional with evaluation and data analysis skills to establish and monitor patient care. This person will work collaboratively on several company projects focused on supporting the Patient Care Network Center. This person is highly motivated and skilled to work at any practice location. The Patient Care Coordinators will apply your practical knowledge to the creation of care programs for patients that need Transitional Care Mangement (TCM) and potential other services such as RPM and CCM. This position combines both practical and administrative aspects of nursing. In this role, you will collect patient health information through the Next Gen platform to help identify risks, provide education and encourage compliance of treatment. Under the direction of the Primary Care Physician (PCP), the Manager- Patient Care Coordinator will also be responsible for creating patient Care Plan that provide goals and interventions to implement, monitor and support the needs of the patients as defined by the PCP This person will help develop NexGen systems and processes to be adapted by the care coordinators to help monitor and reduce chronic disease. The Patient Care Coordinator is responsible for planning and implementing workflows and evaluation activities; the identification, analysis and interpretation of healthcare process to support platform needs; and produce data to summarize findings, support platform and program decisions and meet deliverables. The Patient Care Coordinator will report to Manager of Patient Care Network Center and coordinate her/his work closely with other company sales and operations staff. The Patient Care Coordinator is a high-impact role responsible for monitoring all the patient customers nationwide in the company and act as a point of contact for all practice customer facing inquiries. Critical to the success of this role is the candidate's ability to (a) successfully maintain workflow and processes among patient care coordinators (b) proactively influence customers engagement with action plan based on their analytics data, (c) translate customer clinical and process challenges into innovative Ceras offerings that achieve defined business outcome (process improvement, new services innovation, end-user client analytics and business insight) and (c) collaboration with the product team to rapidly evolve the platform that provide customers the most efficient system to deliver the highest quality of care experience to their members. The powerful combination of the candidate experience with healthcare, patient care management processes, advanced digital platforms should deliver significant results measured by increased member acquisition, member engagement on the platform, and revenue growth for customers and Ceras. Ideal candidate will have startup hands on mindset, strong bias for action, be a thoughtful, creative thinker who gets things done; the technical depth to understand the engineering and product development process and entrepreneurial mindset that enables him/her to build a business case for how the company should take advantage of a customer(s) opportunity.JOB FUNCTIONS:

  • Onboard new patients to the Next Gen platform for Transitional Care Managment (TCM) Services.
    • Provide one-on-one attention to patients on the Next Gen platform
      • Develop curtailed care plans for both the provider and patient
        • Plan and develop, as well as implement and direct, a program that provides services for chronic disease management.
          • Coordinate a database of statistics and narratives for tracking progress and efficacy of treatments and program outcomes with Next GEn Platform
            • Provide direct patient care within the parameters of the program you have developed.
              • Educate, manage and train staff on new methods of approach for chronic disease management through the Next Gen Platform
                • Collaborate with implementation and customer operations team to train users w/in and outside of the practice.
                  • Work with implementation to ensure smooth onboarding experience for all users on the Next Gen Platform
                    • Collaborate with or partners technology team to help with evolve the patient care and coordination features for the platform
                      • Become subject matter expert for patient care management and care coordination activities.
                        • Strong ability to exercise initiative, judgment, organization, problem-solving and decision-making skills.
                          • Skilled in developing and maintain effective relationship with team members and patients.
                            • Capable of organizing work to achieve daily/weekly goals and objectives.

                                • REQUIRED: Certified Medical Assistant( CMA), Licensed Practical Nurse (LPN) or Registered Nurse RN
                                  • 3+ years' experience, with at least 1+ years' experience in treating patients with chronic conditions.
                                    • Experience in case management
                                      • Relevant experience conducting program evaluation.
                                        • Proficiency in collaborative tools such as word, excel, power point and Access.
                                          • Excellent verbal and written communication skills.
                                            • Experience in handling multiple projects and deliverables within deadlines.
                                              • Experience working with multi-disciplinary teams and stakeholders.
                                                • Commitment to continuous learning and improvement
