CHP Home Care & Hospice is hiring a part-time or full-time
Licensed Social Worker (LSW) OR Licensed Independent Social Worker
(LISW) to assist and counsel home care and hospice patients and
families with health-related financial, social, and emotional

patients.*What You Will Do*The *Licensed Social Worker (LSW)* is a
valued member of an interdisciplinary team who provides diagnostic
evaluations of the psychosocial problems of the patient/family
unit, and social service goals for alleviating these problems, and
helps to provide both the counseling and casework needed to meet
these goals.The *Licensed Independent Social Worker (LISW)* is a
valued member of an interdisciplinary team whose primary function
is to oversee our medical social services and staff.*LSW Job
Responsibilities*** The Licensed Social Worker (LSW) performs the
initial psychosocial, emotional, spiritual, and bereavement
assessments in the development and implementation of a
goal-directed, interdisciplinary, care plan.* Conducts ongoing
assessments of patient/family needs and counseling as required*
Provides short-term crisis intervention and individual or family
counseling when indicated* Delivers services under the direction of
a physician who approves the plan of care* The Licensed Social
Worker (LSW) observes, assesses, and brings information to ID team
meetings regarding psychosocial, emotional, spiritual, physical,
and financial conditions affecting the patient and family* Assumes
the active role of advocate for the patient/family unit* Develops
and maintains contact with appropriate community agencies and
services in order to promote interagency cooperation and facilitate
related referrals* Documents comprehensive psychosocial, emotional,
and spiritual assessments clearly and concisely in a timely manner*
Provides education to the patient and family related to issues of
death, dying, and bereavement as needed*LISW Job Responsibilities**
The Licensed Independent Social Worker (LISW) reports to the VP of
Clinical Services and works closely with nursing supervisors,
hospice coordinators, and medical social workers* Keeps social
workers up-to-date with all regulation, policy, and procedure
changes* Responsible for assuring peer-to-peer annual in-home
direct supervisions are being completed* Will inform nursing
supervisors if issues or concerns arise related to patient care or
the inability to cover visits* The Licensed Independent Social
Worker - LISW will see patients as directed by the nursing
supervisor or physician in accordance with each patient's plan of
care* Observes, assesses, and brings information to ID team
meetings regarding psychosocial, emotional, spiritual, physical,
and financial conditions affecting the patient and
family*Qualifications** Bachelor's degree from a school of social
work accredited by the Council on Social Work Education* Minimum of
one year of social work experience in a healthcare setting*
Currently licensed in the State of Ohio and maintain required CEs*
Ability to work independently, make accurate, and at times, quick
judgments* Must have effective communication skills and the ability
to effectively follow instructions* Dependable with a cooperative,
