Position DescriptionPosition Title: Staff Nurse-LPNDescription: Perform family centered nursing services utilizing the nursing process under the direction of the Provider.Classification: HourlySupervision: Reports to the Director of NursingCompletion of all trainings and educational programs is a requirement of employment.Responsibilities:Maintains strict patient confidentiality at all times. Performs patient observations utilizing the nursing process.Documents all care provided according to policy.Follows plan of care formulated by the Provider.Administers medications and injections as prescribed by the Provider.Follows protocols.Follows standard precautions.Performs treatments, procedures as prescribed by the Provider.Performs technical skills.Responds to patient/family/caregiver needs.Responds to crisis situations according to policy.Utilizes supplies in a cost-effective manner.Performs clinic and outreach related tasks.Participates in outcomes Performance Improvement Program.Demonstrates effective communication skills.Establishes and maintains professional working relationship with other members of the health care team.Participates in planning and problem solving.Participates in continuing education and professional growth.Attends staff meetings.Maintains knowledge of, and complies with, personnel, nursing, medical and clerical policies and procedures.Assists with seasonal programs as needed.Assists other team members.Performs room setup.Evaluation of nursing equipment and reports to maintenance when repairs are needed.Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA. OSHA, OIG, guidelines, other State and Federal laws). This also includes reporting to the Board of Directors, Compliance Officer, Privacy Officer, and supervisor any suspected unethical, fraudulent, or unlawful acts or practices.Performs other duties as assigned.Essential Functions:Physical factorsLifting: Up to 30 pounds 4-5 times a day 5-10 pounds routinely.Carrying: Up to 30 pounds 5-10 times a day 5-10 pounds routinely.Standing: 1-2 hours at a time 4-6 hours a day.Walking: 5-10 minutes at a time throughout the day.Sitting: 15-30 minutes at a time 1-2 hours a dayDriving: 15-30 minutes at a time throughout the day.Vision: Color distinction necessary.Hearing: No greater loss than 40dB average for 500, 1000, 2000 HzQualifications:Graduate of an accredited nursing school.Current license to practice as a Licensed Practical Nurse in the State of Ohio.Medication card from approved provider.Two years full time experience as a licensed Practical Nurse.Must have reliable transportation, current driver's license, and provide proof of valid auto insurance.Computer experience is required.Excellent oral and written communication skills.Must be able to multi-taskMust be well organized and detail oriented.Ability to work with a culturally diverse group of people.Must be willing to work flexible hours as required by positionCHS is dedicated to providing a healthy environment for our patients, families and staff.
