Job Description:

1. Direct Nursing Care in Accordance with Established Policies, Procedures, and Protocols : Implementation of Care : This includes administering medications, performing treatments, assisting with daily activities (e.g., bathing, dressing), and addressing any acute or chronic needs based on the patient’s condition. Compliance : Ensuring that all actions and interventions align with the facility's protocols and legal standards, such as infection control measures, safety procedures, and documentation requirements. 2. Implement and Monitor Patient Care Plans : Assessment and Planning : Developing individualized care plans based on initial and ongoing assessments, identifying patient goals, and documenting required interventions. Monitoring : Continuously evaluating the effectiveness of the care plan, adjusting as necessary based on the patient’s response, lab results, or physician recommendations. 3. Monitor, Record, and Communicate Patient Condition : Observation : Routinely assessing vital signs, laboratory results, mental state, and overall patient well-being. Documentation : Recording any changes in condition, adverse reactions, or improvements in the patient’s health, ensuring all relevant information is updated in the medical record. Communication : Regularly communicating patient status with the healthcare team to ensure coordinated care, discussing any concerns, or reporting significant changes to a physician or team lead. 4. Serve as the Primary Coordinator of All Disciplines for Well-Coordinated Patient Care : Team Leadership : Collaborating with doctors, physical therapists, dietitians, social workers, and other healthcare professionals to ensure a holistic approach to patient care. Coordination of Care : Making sure that patient interventions from different disciplines are not duplicated, that the patient’s needs are met, and ensuring smooth transitions between different phases of care (e.g., from acute care to rehabilitation). 5. Note and Carry Out Physician and Nursing Orders : Order Review and Implementation : Carefully reviewing and acting upon physician orders, whether for medications, treatments, or tests, ensuring accurate and timely execution. Collaboration with Physicians : Communicating any concerns or clarifications needed about orders to the physician, ensuring a shared understanding of the patient’s needs and plan. 6. Assess and Coordinate Patient's Discharge Planning Needs with Healthcare Team : Discharge Planning : Early identification of discharge goals, such as patient education, home care needs, follow-up appointments, or necessary equipment. Multidisciplinary Team Approach : Coordinating with social services, case managers, and family members to ensure a seamless discharge process and that the patient has the necessary resources at home.
