Job Description & Requirements - Specialty: Primary Care - Discipline: Nurse Practitioner - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Local Contract Job Title: Primary Care Nurse Practitioner Pay rate: $50/hr - $60/hr $5000 Sign on Bonus Location: Defiance, Ohio 43512 Job Type: Full-Time/Permanent Schedule: Monday–Friday (Day Shift) Job Description We are currently seeking a skilled and compassionate Primary Care Nurse Practitioner to join our team in Defiance, Ohio. This is a full-time outpatient role focused on providing primary care services to adult and geriatric patients. The ideal candidate will bring strong clinical expertise in adult-gerontology and a passion for delivering patient-centered care. Key Responsibilities: - Provide primary and preventative care for long term care patients residing in skilled nursing facilities. - Manage Residents with acute change in condition. - Advanced care planning and chronic care management. - Prescribe appropriate medications, including controlled substances in accordance with OH Law. - Order, review and/or follow-up with treatments and diagnostic testing. - Communicate with the nursing home staff on a regular basis and as needed to discuss changes in condition.and/or the care plan. - Collaborate with a primary care physician.
