Their team of physicians, nurse practitioners, behavioral health providers, and staff deliver friendly, expert care to individuals and families. Requirements : Active License in The State of Indiana DEA Certification Board Certification or Board Eligibility in Family Practice or Internal Medicine Responsibilities : Flexible Schedule. Monday –Friday: Mon and Tue: 7:30a – 6p, Wed and Th 7:30a-7p, Fri 7:45a-5p 4 or 5 Days a Week - At Least 36 Hours 20Patients a Day Call by Phone Every 2 Months Seeing Patients of All Ages No Surgery, Standard Procedures
