incredible Skilled Nursing Facilities in OH is looking for a
superstar Wound Care Nurse Practitioner to be part of a
facilities-based team of medical providers who provide medical
services to patients living in long-term care facilities. While an
interdisciplinary care team to provide medical care to patients in

Current, unrestricted NP or PA license in the state of OH. Board
Certification. DEA licensure. Clinical background in wound care.
Experience working in a long-term care setting. Ability to work
with an EMR. Ability to manage patients with advanced illness and
multiple chronic conditions. Ability to work within an
interdisciplinary care team to coordinate patient care. Wound care
experienceThe job description of a Wound Care Nurse Practitioner
includes conducting physical examinations, Provide wound care for
long-term care patients residing in skilled nursing facilities. As
a Nurse Practitioner, youll have to work closely with other
healthcare professionals, so you should have a team spirit and a
positive attitude. Patience, problem-solving skills, and
communication ability are important.Responsibilities include but
are not limited to: Prescribe appropriate medications . Order,
review, and/or follow-up with treatments and diagnostic testing.
Communicate with the nursing home staff on a regular and as needed
to discuss changes in condition. and/or the care plan. Collaborate
with primary care physician. Provider will assess and monitor
residents on a regular basis. Provider will be available to assist
staff in assessing wound and pressure ulcer risks. Provider will
evaluate multiple factors related to wounds: i.e. Nutrition,
Mobility, Incontinence associated dermatitis, Pressure ulcers,
Diabetic ulcers, Arterial ulcers, Venous ulcers, and Common
dermatologic conditionsMANAGEMENT Measures to decrease the risk of
wounds with recommendations to staff. Measures to optimize healing
via protocols Superficial wound debridement Dressing selection and
management Placement of negative pressure wound dressings Post
